Forms BUG: Empty QUERY_STRING env var

The most common problem is not selecting the correct "type of information" button in Step 1. The second most common problem is specifying a city that does not have an airport. Our database is from the FAA so it only has cities with airports. In the US only airports that operate a beacon from sunset to sunrise are included.

Other common problems are with date & latitude/longitude formats. While several formats are understood, the following are suggested:

"5 1", "5/1", "May 1", or "1 May" for May 1 of the current year.

The year may be appended with or without an initial "19" or "20" and with a space and/or common date punctuation.

Valid dates include 03/21/1999, 03/21/2000, 03/21/00, and March 21 2000 Valid years are 1951 through 2030.

"N40.50 W90.25", "N40-30 W90-15", "N40-30-0 W90-15-00"
for North 40 degrees 30 minutes by West 90 degrees 15 minutes.
